What to Expect During the First Visit at the Goldman Vein Institute
Patients visiting any of our convenient South Florida locations for the first time should arrive at least 15 minutes prior to the scheduled appointment to ensure that all paperwork is complete and accurate.
Patients visiting the Goldman Vein Institute for the first time are requested to please bring the following information:
• Completed Office Policy form
• Any pre-registration patient forms
• Health insurance cards
• Photo ID
• Current list of any medications
• Co-payment, if required
